Preparing for your first visit
When visiting us for the first time, please bring the following items with you:
- Insurance card
- Driver’s license or other state-issued identification
- New patient forms
In order to make your first Magnetic Resonance Imaging (MRI) experience as fast and simple as possible, we have provided downloads for all the necessary forms. You can fill them out in the office, or you can complete them at home to make your visit faster. Please call us with any questions or concerns while preparing these forms – we’re here to help from 7:00 am to 7:00 pm on business days.
