Parkwood Open Imaging is committed to protecting the privacy of your health information. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) and your rights concerning that information under the Health Insurance Portability and Accountability Act (HIPAA) and the Michigan Medical Records Access Act.

You have the right to inspect and secure a comprehensive copy of your protected health records and imaging files. In accordance with the Michigan Medical Records Access Act, we may apply a reasonable, statutorily aligned fee to manage retrieval and media replication costs.

Right to Request Amendment

You hold the right to request that our team amend historical health data if you demonstrate that the active documentation is factually incorrect or incomplete.

Right to an Accounting of Disclosures

You maintain the right to request an official structural log of certain healthcare disclosures executed by our facility outside routine treatment, payment, or operational tasks.

Right to Request Restrictions

You possess the right to request specific restrictions on how we deploy or share your medical files for treatment, payment, or facility operations. While we evaluate all restriction requests, we are not legally bound to agree to every custom limitation.

Right to Confidential Communications

You have the right to request that our facility connect with you regarding personal medical matters via specific communication paths or alternative regional locations.

Right to a Paper Copy

You maintain an absolute right to acquire a physical paper copy of this privacy notice at any time upon simple request to our front desk.

We reserve the legal right to adapt the terms of this Notice, applying the updated privacy safeguards across all historical and active medical records maintained by our facility. Revised notices will be posted within our patient waiting rooms and updated across our digital website infrastructure.

Complaints and Privacy Appeals

If you believe your personal health privacy rights have been compromised by our facility, you may file a formal complaint with our internal Privacy Officer or route an appeal directly to the Secretary of the Department of Health and Human Services. We strictly prohibit any form of institutional retaliation against patients who exercise their right to file a complaint.
